In hospitals, patient discharge is typically a complex orchestration between doctors, nurses, case managers, social workers, and post-discharge providers. A 200-bed county hospital in Northern California found that as the only trauma center serving a “medically underserved” county, it needed a streamlined discharge process that not only smoothly facilitates patient flow through the hospital, but also provides patients with the proper support in the community to prevent readmission.
